Primary Orthostatic Tremor, also known as Orthostatic Tremor (OT), is a progressive neurological movement disorder, characterized by high frequency tremors, predominantly in the legs when in a standing position, and an immediate sense of instability.
Orthostatic Tremor. Orthostatic tremor (OT) refers to a high-frequency (13 18 Hz) tremor of the legs upon standing. Rarely, trunk and abdomen may be involved. When OT is the only clinical feature, i.e, an isolated tremor syndrome, it is termed a primary OT.

Orthostatic Tremor . In orthostatic tremor, the legs and trunk start trembling soon after standing. Someone with orthostatic tremor may not notice a tremor so much as unsteadiness after standing up. It improves as soon as someone sits down. Treatment can be with either Klonopin (clonazepam) or Mysoline (primidone).
Orthostatic tremor is a progressive disorder with increased disability although tremor frequency is unchanged over time. In most cases, orthostatic tremor represents an isolated syndrome. Drug treatments are unsatisfactory but surgery may hold promise.

Management of primary orthostatic tremor (POT) remains challenging, and medication is often ineffective.We report the case of a 53-year-old female with orthostatic tremor for 6 years who was refractory to gabapentin, clonazepam, primidone and propranolol.
